Diversion Case Manager
Quincy, MA

SUMMARY:
Our client is seeking a Diversion Case Manager. The Diversion Case Manager is responsible for compliance with requirements and implementation of services as specified in the contract and by Housing Program Director.

ESSENTIAL DUTIES & RESPONSIBILITIES:
  1. Understand Diversion contract contents, including goals and program requirements to ensure compliance.
  2. Attend all meetings that focus on Diversion services, placements, etc. at organizations as determined by Housing Program Director.
  3. Determine use of financial assistance resources for all clients in coordination with other Housing Staff, Diversion Managers and Housing Program Director.
  4. Work with clients to create a viable re-housing plan, using all strategic resources.  Assist clients in housing search, including private and public housing opportunities. Keep records of applications filed and ensure that clients keep all applications updated.
  5. Provide transportation to clients for apartment viewings/ landlord meetings/ voucher information sessions, etc. as needed.
  6. Adhering to WISP policies and maintaining complete client files, including client release forms, and personal verification documents is an essential job duty.
  7. Enter all required client information and outcomes into databases and State databases per program requirements.
  8. Provide basic financial literacy counseling services to all clients including budgeting and understanding credit. Review Fair Housing Laws  - both Federal and State with all families to ensure their protection under the statutes.
  9. Provide stabilization services per the Stabilization plan to families after placement into permanent housing and assist clients in accessing area resources to meet their needs and keep them housed. Meet with clients regularly to assess progress toward goals and modify stabilization plans when necessary.
  10. Ensure that clients are connected to all available resources, including but not limited to: SNAP Benefits, Food Assistance, Energy Programs, Adult Education, ESOL Classes, Head Start, Community Care for Kids, Volunteer Income Tax Assistance/Earned Income Tax Credit, Financial Literacy Services, Health Insurance Enrollment and Job Training. Assist clients with all applications as necessary.
  11. Any other duties deemed necessary by Housing Program Director.
QUALIFICATIONS/SKILLS:
  1. An Associate’s Degree or equivalent plus 2 years’ experience in the affordable housing industry, or Bachelor’s Degree in related field plus one year experience in the affordable housing industry.
  2. Demonstrated ability to conduct effective client interviewing, to develop client case management plans and to document plans and client outcomes.
  3. Demonstrated knowledge of community resources available to low-income households including knowledge of basic housing law and affordable housing resources.
  4. Ability to understand and interpret complex government guidelines and regulations.
  5. Excellent computer skills, including experience working with databases and Microsoft office applications.
  6. Excellent time management skills including strong communication and interpersonal skills.
  7. Ability to transport clients requiring a valid driver’s license and properly insured/registered car required.
  8. Commitment to and ability to work with sensitivity to the complex needs of low-income clients.
  9. Demonstrated commitment to working on behalf of and effectively with diverse individuals and organizations.
