Quincy, MA

Established non-medical, Homecare Company has an immediate full-time opening for a Shared Living Case Manager.

Job Summary

Primarily responsible for overall service coordination, care management, and scheduling of case load.

Responsibilities Include
  • Providing ongoing supervision of caregivers as assigned by Department Supervisor. Performing regularly scheduled performance appraisals with development planning.
  • Providing all required and requested administrative and fiscal documentation for consumer service and program operation.
  • Providing case management services to a consumer case load assigned by department Supervisor.
  • Developing Individual Support Plans in conjunction with funding source and in accordance with corporate and regulatory standards. Coordinating plan implementation.
  • Maintaining complete and current consumer and reporting records in accordance with corporate and regulatory standards. Assisting with all legal, licensing, regulatory and certification activities as needed.
  • Assisting and following pre-service and in-service training to caregivers as assigned. Assisting with provider training planning and development.
  • Assisting the department supervisor with the provider selection, training, retention and social activities. Participating in all activities designed to enhance and build an active network of caregivers.

Qualifications and Experience
  • Bachelor's degree in the human services or rehabilitation field.
  • Minimum three years direct care experience in developmental disabilities, mental health, child care or adult services.
  • Strong organization skills.
  • Excellent communication skills.
  • Valid driver’s license and ability to travel (locally) to perform essential functions of position.
