We are currently seeking a Social Worker for a long-term, temporary assignment in Canton! The Social Worker provides a range of individualized services to patients with significant medical and cognitive disabilities and their caregivers. The Social Worker is responsible for psychosocial assessments, identifying health-related social needs for each family on the assigned caseload. They are responsible for planning and implementation of the patient’s treatment and discharge plan, which reflects the patient’s social work needs from admission to discharge, and which requires added collaboration and coordination responsibilities.
Manage a caseload of medically complex pediatric patients.
Complete psychosocial assessments in line with established standards.
Build relationships with patients and families to assess needs and improve communication.
Participate in admissions and provide ongoing caregiver support.
Develop and coordinate discharge plans with community providers and agencies.
Support patient transitions and educate caregivers on discharge processes.
Oversee guardianship processes when needed.
Evaluate patient/family progress and revise treatment plans as necessary.
Serve as liaison to the Department of Children and Families for patients in custody.
Participate in team meetings, consultations, and performance improvement activities.
Complete timely and accurate documentation in the electronic health record.
Travel to appointments and community resources as required.
Qualifications:
Ability to mentor, prioritize cases, and handle emergencies.
Knowledge of community resources and family dynamics.
Proficiency in computer tasks and effective communication.
Ability to multi-task, prioritize, and maintain accurate records.
Sound judgment in maintaining professional boundaries and confidentiality.
Requirements:
Bachelor’s or higher degree in a relevant human services field (e.g., social work, psychology, sociology) or a Master’s degree in a relevant field may substitute for one year of experience.
A current and valid Licensure as a Licensed Social Work Associate, Licensed Social Worker, Licensed Certified Social Worker, or Licensed Independent Clinical Social Worker issued by the Massachusetts Board of Registration
3-5 years of experience in social work, with pediatric experience preferred.
Valid driver’s license as required.
